The Patient mentors Project,
provides support and assistance in
the development of user lead self-management
programs for people experiencing chronic
illnesses, such as coronary heart
disease, asthma, arthritis, diabetes
etc.
The project, which is free to its
users, is aimed at helping to develop
coping skills for people with common
issues such as pain and stress as
well as addressing the negative knock
on effect that chronic health can
have upon income, employment and social
connection.
The project includes the development
of a network of local sufferers from
chronic illnesses, able to act as
skilled self-management tutors/mentors
in relation to issues commonly faced
by people with chronic illnesses.
These tutor/mentors run self-management
programs for other local people with
chronic conditions enabling them to
develop knowledge, skills and support
in the management of their own conditions.
